The Administrative Support Section issues EPA identification numbers for generators, transporters, recyclers, used oil handlers and universal waste handlers. The section reviews annual reports and assessment returns from hazardous waste generators and facilities. We also conduct data management for the hazardous waste program. In addition, we review financial assurance submittals from hazardous waste treatment, storage or disposal facilities.
The programs that are coordinated by the Administrative Support Section are:

Notification of Hazardous Waste Activity Form (DEP 7037 - revised July 1996) - two pages.
1. To obtain an EPA identification number, modify current registration or request approval to treat hazardous waste on-site during the generator accumulation period.
2. For hazardous waste generators (small or large), transporters and recyclers (i.e., precious metals, lead-acid batteries, distillation units, etc.).
3. For used oil marketers, burners, transporters, processor/re-refiners and collection centers.
4. For large quantity universal waste handlers.
5. For courtesy EPA identification numbers (non-hazardous wastes like PCBs, household hazardous waste collection centers, etc.).
Request to be Removed from the Hazardous Waste Handler List (DEP 7086 - revised July 1996) - one page.
To eliminate reporting and record-keeping requirements, for companies that no longer generate hazardous waste.

The Hazardous Waste Annual Report consists of six separate forms:
1. Form #1 - Identification and Certification (DEP 7072 revised October 2005) - one page. ALL large and small quantity hazardous waste generators and treatment, storage and disposal facilities must complete this form.
2. Form #2 - Waste Generation and Management (DEP 7072 revised October 2005) - one page. Anyone who generated and/or shipped hazardous waste off-site during the report year must complete this form for each waste stream.
3. Form #3 - Waste Received from Off-site (DEP 7072 revised October 2005) - one page. Anyone who received hazardous waste from off-site must complete this form.
4. Form #4 - Waste Treatment, Disposal and Recycling System (DEP 7072 - 91 revised July 1996) - two pages. Anyone who treated, disposed or recycled hazardous waste on-site must complete this form.
5. Form #5 - Summary of Waste Shipped Off-site (DEP 7072 revised October 2005) - one page. Anyone who shipped hazardous waste off-site must complete this form.
6. Form #6 - Summary of Waste Received by your TSD Facility (DEP 7072 revised October 2005) - one page. Anyone who received hazardous waste from off-site must complete this form.
7. Detailed Instructions for Completing Annual Report forms - eight pages.
8. County Judge/Executive List - identifies each county judge/executive in Kentucky and his or her mailing address - seven pages.
Supplement to the Annual Report Forms - contains the code lists for completing the annual report forms including: activity codes, form codes, handling codes, source codes, system type codes and conversion rates chart - five pages.

1. Groundwater Sample Analysis Form (DEP 8046, effective August 1995)-two pages. For all hazardous waste facilities monitoring groundwater to submit analysis results.
2. Hazardous Waste Groundwater Report Form (DEP 8046A, effective March 1996) - two pages. For all hazardous waste facilities monitoring groundwater to submit facility-specific (historic) information about the site.
