Kentucky Manifest Requirements

The U.S. Environmental Protection Agency (EPA) revised the Hazardous Waste Manifest Forms used to track hazardous waste from a generator’s site to the site of its disposition. Handlers of waste will obtain new forms from any source that has registered with EPA to print and distribute the form. The new Uniform Hazardous Waste manifest forms must be used beginning Sept. 5, 2006. This Web page explains Kentucky’s manifest requirements.

How do I submit manifest copies to Kentucky?
Kentucky does not currently require that a copy of the manifest be sent to the state. If you need to send a copy for your business rules, submit them to the contact listed below.

What are Kentucky specific waste codes?
N001 – GB (isopropyl methyl phosphonofluoridate) (H)

N002 – VX (o-ethyl-S-(2-diisopropyl-aminoethyl)-methyl phosphonothiolate) (H)

N003 – H   (bis(2-chloroethyl) sulfide) and related compounds (H)
