File Room

The UST Branch's file room maintains an accurate historical record for the general public to reference.  Items found in the file room consist of all correspondence from the tank owner to the UST Branch as well as all information submitted regarding the facility.  In addition, information concerning the eligibility, approval and reimbursement through the Petroleum Storage Tank Environmental Assurance Fund is found in the UST Branch's file room.

Anyone may request an inspection of the UST Branch's records.  Unless the material is exempt from disclosure pursuant to KRS 61.878, this office must respond to a request within three working days from the date of receipt of the request. The request should contain all information known about the property/facility in question, e.g., property/facility owner, facility address, facility name, facility identification number, agency interest number, owner file number and/or the application for assistance number, if known. 

Kentucky law requires citizens to complete all requests to view public records in writing. The Open Records Request form may be downloaded either in the PDF or MS Word format. Open records requests may be made by fax 502-564-0094 or via e-mail. The free Adobe Acrobat Reader to view PDF documents is available for download here.                                        

Copies may be made at the UST Branch office during a file review at a cost of 10 cents per page or you may request a list of off-site vendors that will come in and copy the file information for you at their additional charge.
